1911 Census:
Samuel Davis, aged 32, was employed as a bridge painter. He resided at 19, Kensington Street, Derby, and he was the husband of Emily Davis, and father of Emily, Alice, Samuel and Albert Davis.

Service record:
Samuel Davis enlisted on the 22nd November 1915, giving his home address as 24, Kensington Street, Derby, his age as 37 and his occupation as painter. He was given the regimental number of 5922, and originally joined the 2nd/5th Battalion. He was 5 feet 3.5 inches in height, had tattoo marks on both forearms and the terminal phalanx of one of his thumbs was missing.
He served in France from the 26th February 1917, until he received a gunshot wound to the buttock on the 4th May, for which he was evacuated to England for treatment in hospital. When he had recovered from his wound, he was sent back to France on the 4th January 1918, joining the 1st Battalion on the 9th. He served with them until being killed in action on the 22nd April.
Personal effects sent to his wife after his death included a card, photographs, a strop, some shears, two pairs of scissors and a notebook.
